Travel CNA – 10-Hour Day Shifts – Colorado Springs, Colorado

Connected Health Care is partnering with a reputable healthcare facility in Colorado Springs, Colorado to find a Travel CNA for 10-hour day shifts. This is an exciting opportunity for compassionate and skilled Certified Nursing Assistants to provide care to hospice patients in their homes while enjoying the beautiful surroundings of Colorado Springs.

Job Description:

• Travel to patients’ homes to provide hospice care, including assisting with daily activities such as bathing, dressing, and mobility.
• Provide comfort and support to hospice patients, ensuring their dignity and quality of life in their final stages.
• Monitor patients' vital signs and report any significant changes in their condition to the healthcare team.
• Assist with medication reminders, light housekeeping, and other tasks as needed to support patients and families.
• Travel between patient homes, which are located approximately 45 minutes apart, ensuring timely and efficient care.
• Document patient care, activities, and progress in the EMR (Electronic Medical Record).
• Work collaboratively with family members, patients, and other healthcare professionals to ensure the best care possible.

Education & Certification Requirements:

• CNA Certification through DORA (Department of Regulatory Agencies) required.
• Flu shot required.
• Clear background check required.

Required Experience:

• Previous CNA experience, especially in hospice or home care, is preferred but not required.
• Must be able to independently travel between patient homes and provide care in various home environments.
• Strong communication and interpersonal skills for interacting with patients, families, and healthcare teams.
• Ability to work efficiently and responsibly with minimal supervision.
