Child and Adolescent Case Manager - Floresville, TX
Connected Health Care, a leading recruitment and staffing firm, is seeking a Child and Adolescent Case Manager for our client in Floresville, TX. This is an excellent opportunity for an experienced mental health professional to make a meaningful impact on the lives of children and adolescents in a supportive and collaborative environment.
Position Details:
- Schedule: Monday through Friday, 8:00 AM - 5:00 PM
- Contract Duration: 13 to 26 weeks
- Weekly Hours: 40 hours per week
- Compensation: Up to $1120 per week, based on experience
Responsibilities:
- Coordinate and manage cases for children and adolescents, ensuring access to appropriate services and resources
- Develop and implement individualized care plans, ensuring the needs of clients are met effectively
- Provide crisis intervention and support in emergency situations
- Monitor and assess clients' progress, adjusting care plans as necessary
- Collaborate with other professionals, including counselors, social workers, and healthcare providers
- Maintain thorough documentation of services provided, case progress, and communication with families and clients
- Advocate for clients, ensuring they receive necessary services and support
- Provide psychoeducation to clients and families to enhance understanding of mental health conditions
Education and Certification Requirements:
- Bachelor’s degree in Psychology, Social Work, Medicine, Nursing, Rehabilitation, Counseling, Sociology, Human Growth and Development, Physician Assistant, Gerontology, Special Education, Educational Psychology, Early Childhood Education, or Early Childhood Intervention
- Must hold QMHP (Qualified Mental Health Professional) certification
- Minimum of 2 years of experience in mental health
- At least 1 year of experience working with children and adolescent populations
- Higher compensation is available for bilingual candidates (English/Spanish) and those with prior experience as a case manager in mental health, social, behavioral, or human services
