Job Responsibilities
  • Provide comprehensive social work services to residents in long-term care facilities, including assessments, care planning, and coordination of services.
  • Collaborate with multidisciplinary teams to develop and implement individualized care plans that meet residents' psychosocial needs.
  • Maintain accurate and timely documentation of all client interactions, assessments, and treatment plans in compliance with facility protocols.
  • Offer counseling and support to residents and their families to address emotional, psychological, and social challenges.
  • Execute discharge planning and coordinate community resources to ensure continuity of care post-discharge.
