- Certifications Requirements: Licensed Clinical Social Worker (LCSW) or equivalent certification; CPR certification; State license in Minnesota.
- Experience Requirements: Minimum of 1 year of experience in Long Term Care (LTC).
- Other Requirements: Ability to work 5x8 shifts (8 am - 5 pm); excellent communication skills; ability to work independently; reliable transportation outside an 80-mile radius; proficient in EMR systems; Ability to handle physically demanding duties; strong interpersonal skills.

Job Responsibilities
- Assess and develop individualized care plans for patients in long-term care settings, ensuring compliance with facility policies and patient needs.
- Collaborate with healthcare teams, residents, and families to coordinate services and provide social support and counseling.
- Maintain accurate documentation of patient interactions, progress notes, and treatment plans within electronic medical records (EMR) systems.
- Provide crisis intervention and emotional support to residents experiencing behavioral or emotional difficulties.
- Execute discharge planning and coordinate community resource referrals to support patient continuity of care post-discharge.
