- Provide comprehensive social work services to residents in SNF facilities, including assessments, care planning, and discharge planning.
- Collaborate with interdisciplinary team members to develop individualized care plans tailored to resident needs.
- Document all patient interactions, assessments, and treatment plans accurately and timely in the EMR system.
- Maintain compliance with all federal, state, and facility policies and regulations regarding patient care and documentation.
- Participate in team meetings, case reviews, and ongoing training to ensure quality care delivery.
