- Experience Requirements: At least 1 year of LTC experience in a Skilled Nursing Facility
- Other Requirements: Ability to work 8-hour days, reliable transportation to facility outside 80-mile radius, strong communication skills, ability to work independently

- Assess the psychosocial needs of residents and develop appropriate care plans in collaboration with interdisciplinary teams.
- Provide individual counseling and support to residents and their families to address emotional and social concerns.
- Maintain accurate documentation of services provided, resident progress, and care plans in compliance with facility protocols.
- Collaborate with nursing staff and healthcare providers to coordinate resident care and ensure compliance with regulatory requirements.
- Participate in staff meetings and training sessions to stay current with facility policies and best practices in social work.
