Full-Time Home Health Registered Nurse (RN) - Springfield, Missouri
Compensation: Up to $40/hr (Based on Experience)
Schedule: Full-Time | Monday – Friday 8Am - 5PM

Connected Health Care, a trusted recruitment and staffing partner, is seeking a compassionate and experienced Home Health Registered Nurse to join a top-tier healthcare client in Springfield, MO. This is an excellent opportunity for a nurse who values autonomy, flexibility, and meaningful one-on-one patient care in a supportive team environment.

Job Description:

  • Provide skilled nursing care to patients in their homes within the designated service area

  • Conduct initial assessments, develop care plans, and perform regular patient visits

  • Administer medications, monitor vital signs, and document patient progress

  • Educate patients and families on disease management, medication administration, and home safety

  • Collaborate with physicians, case managers, and other clinicians to ensure continuity of care

  • Maintain accurate and timely documentation in compliance with agency protocols and regulations

  • Promote positive health outcomes and patient independence in the home setting

Education and Certification Requirements:

  • Active RN license in the state of Missouri (or compact state)

  • Graduate of an accredited Nursing program (ADN or BSN)

  • BLS certification required

  • Prior home health experience strongly preferred

  • Strong assessment, communication, and critical thinking skills

  • Reliable transportation and valid driver’s license required
