Job Description: As a Physical Therapist with Connected Health Care, you will assess, plan, and implement rehabilitative programs that improve mobility, relieve pain, and enhance the overall quality of life for patients in the LTC setting. Collaborate with interdisciplinary teams to develop comprehensive care plans tailored to each patient's needs. Utilize your expertise to educate patients and their families on therapeutic exercises and techniques to promote independence and recovery.

Education and Certification Requirements:

• Graduate of an accredited Physical Therapy program
• Current PT license in the state of Washington
• General certifications required
