**About the Role:**

As a Physical Therapist, your role is pivotal in providing high-quality, patient-centered care. You will manage the entire process of therapeutic care for a designated group of patients in collaboration with the physician and the patient care team. Your responsibilities include conducting patient assessments and actively participating in the development and implementation of individualized care plans. The focus is on achieving patient goals, fostering patient engagement, and ensuring a positive patient experience. The Physical Therapist will provide or oversee a significant portion of the patient's physical therapy to support them throughout their care journey.

**What You Will Do in This Role:**

1. Provide evaluations and treatment interventions within the legal scope of practice and professional code of ethics.
2. Assess and evaluate therapeutic, rehabilitative, and functional status, actively contributing to the overall care plan.
3. Deliver purposeful and skillful therapeutic care at the patient's location, implementing procedures and techniques to effect changes in condition and function in line with the physician's care plan.
4. Instruct patients, families, and caregivers in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be utilized outside of therapy sessions.
5. Prioritize caseload, determine daily staffing requirements, and coordinate assignments and handoffs.
