Full Time Registered Nurse (RN) – Home Health & Hospice - Chadron, Nebraska
Sign-On Bonus Available
Connected Health Care, a trusted healthcare recruitment and staffing firm, is partnering with a respected healthcare provider in Chadron, Nebraska to recruit a Full-Time Registered Nurse (RN) – Home Health & Hospice. This rewarding opportunity allows nurses to provide compassionate, patient-centered care to individuals in their homes while supporting patients and families during critical moments of care.
The ideal candidate will have strong clinical skills, a compassionate bedside manner, and experience or interest in home health nursing or hospice care. This role offers the opportunity to make a meaningful impact while working in a supportive healthcare environment.
Job Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with individualized care plans
- Assess patient health status, monitor symptoms, and document clinical findings accurately
- Administer medications, treatments, and wound care as prescribed by physicians
- Provide hospice and palliative care focused on patient comfort, dignity, and quality of life
- Educate patients and family members about disease management, medications, and care techniques
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals
- Monitor patient progress and adjust care plans based on changing needs
- Maintain accurate and timely documentation in compliance with regulatory standards
- Support patients and families emotionally while delivering compassionate end-of-life care
- Participate in interdisciplinary team meetings to discuss patient care plans and progress
Education Requirements
- Associate Degree in Nursing or Bachelor of Science in Nursing from an accredited nursing program
Licensure and Certification Requirements
- Active Registered Nurse (RN) license or eligibility for licensure in the state of Nebraska
- Current Basic Life Support certification
- Valid driver’s license and reliable transportation for patient home visits
Preferred Qualifications
- Previous experience in home health, hospice, palliative care, or community health nursing
- Strong patient assessment and care planning skills
- Ability to work independently while managing multiple patient visits
- Excellent communication and interpersonal skills when working with patients and families
- Compassionate approach to patient care with a focus on comfort and dignity
