Job Description:

• Provide compassionate and holistic nursing care to patients receiving hospice services, focusing on pain management, symptom control, and end-of-life care.
• Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs to develop individualized care plans in collaboration with the interdisciplinary team.
• Administer medications, perform skilled nursing procedures, and provide education and support to patients and their families to promote comfort and dignity throughout the hospice journey.
• Coordinate care with physicians, nurse practitioners, social workers, spiritual counselors, and other healthcare professionals to ensure coordinated and effective care delivery.
• Document patient assessments, interventions, and outcomes accurately and timely in accordance with regulatory standards and agency policies.
• Participate in team meetings, case conferences, and in-services to enhance clinical knowledge, promote interdisciplinary collaboration, and improve patient care quality.

Requirements:

• Graduate of an accredited nursing program with a valid Registered Nurse (RN) license in the state of Colorado.
• Minimum of one year of nursing experience in hospice, home health, palliative care, or a related field preferred.
• Current CPR certification; additional certifications such as Certified Hospice and Palliative Nurse (CHPN) are advantageous.
• Strong assessment, communication, and interpersonal skills with a compassionate and empathetic approach to patient care.
• Ability to work independently and collaboratively within a multidisciplinary team environment.
• Proficient computer skills and familiarity with electronic medical records (EMR) systems.
