Connected Health Care is seeking a dedicated and compassionate Full-Time LTAC Registered Nurse (RN) to join our client’s healthcare team in Jamestown, ND. This is an excellent opportunity for an experienced RN to work in a Long-Term Acute Care (LTAC) facility that provides specialized care for patients with complex medical conditions. As an LTAC RN, you will provide high-quality care while working collaboratively with a multi-disciplinary team to improve patient outcomes.
Job Description:
- Assess, plan, implement, and evaluate patient care in a LTAC setting
- Monitor patient vitals, administering medications, and managing treatment plans
- Perform patient care duties including wound care, catheter care, and respiratory management
- Work closely with physicians, specialists, and other healthcare professionals to ensure optimal patient care
- Document patient care in electronic health records (EHR) accurately and timely
- Educate patients and their families about diagnoses, treatments, and care plans
- Ensure compliance with all healthcare regulations and policies
- Participate in team meetings and contribute to care plan development
- Manage care for patients requiring specialized interventions such as dialysis, ventilator support, and wound care
- Demonstrate effective communication skills with patients, families, and team members
Education & Certification Requirements:
- Current and valid RN license in the state of North Dakota
- Graduate of an accredited nursing program
- BLS (Basic Life Support) certification required
