Key Responsibilities:
- Conduct psychosocial assessments and evaluations for patients and their families.
- Provide counseling and support for patients dealing with medical, emotional, or financial challenges.
- Assist with care coordination and collaborate with medical staff to develop discharge plans and ensure a smooth transition of care.
- Provide referrals to community resources such as housing, mental health services, and financial assistance.
- Support patients and families in navigating complex healthcare systems and advocate for patient rights.

- Document all patient interactions and social work interventions in accordance with hospital policies and regulatory requirements.
- Participate in case reviews and care planning meetings with other healthcare professionals.
- Ensure compliance with state and federal regulations related to healthcare services.
Education and Certification Requirements:
- Master's degree in Social Work (MSW) from an accredited university preferred, Bachelors degree required.Â
- LCSW (Licensed Clinical Social Worker) required.
- Valid California state licensure as a Clinical Social Worker.
- A minimum of two (2) years of clinical experience in a hospital or healthcare setting is preferred.
- Basic Life Support (BLS) certification preferred.
Preferred Qualifications:
- Experience in hospital-based social work or medical social work.
- Knowledge of community resources and services available in Bishop and surrounding areas.
- Strong interpersonal skills and the ability to collaborate with healthcare teams.
- Crisis intervention experience and the ability to work in high-pressure situations.
