If you are passionate about providing high-quality, personalized care in the comfort of patients’ homes, we’d love to hear from you.
About the Role:
As a Home Health Physical Therapist, you’ll help patients regain strength and mobility after illness, injury, or surgery—right in their own homes. You’ll create tailored treatment plans, monitor progress, and work closely with nurses, caregivers, and other allied health professionals to ensure holistic, effective care.
Responsibilities:

Conduct in-home assessments to evaluate patient mobility, balance, strength, and overall functional capacity
-
Develop and implement individualized therapy plans
-
Educate patients and caregivers on exercises and strategies to improve outcomes and prevent injury
-
Maintain accurate documentation and communicate regularly with the care team
-
Ensure compliance with all clinical and regulatory standards
Qualifications:
-
Current California Physical Therapist License
-
One or more years of experience in home health (preferred but not required)
-
Strong communication, documentation, and clinical reasoning skills
-
Valid driver's license and reliable transportation
-
Compassionate, self-motivated, and organized
