TRAVEL/LOCAL -Â Hospice Registered Nurse/Case Manager
Qualifications:
- Associate’s Degree in Nursing (BSN preferred).
- Minimum of one year nursing experience (two years preferred).
- Current RN license in Michigan and valid driver’s license.
- CPR certification required, CHPN certification a plus.
- Strong communication, problem-solving, and nursing skills.
Job Overview:
Provide professional nursing care for patients in their homes, coordinating hospice care and managing patient caseloads with a multidisciplinary team. Focus on assessment, care planning, symptom management, and patient education.
Key Responsibilities:
- Maintain hospice policies and procedures.
- Participate in quality improvement and infection control programs.
- Coordinate care, assess patient needs, and develop care plans.
- Implement and evaluate patient care, collaborating with the interdisciplinary team.
- Supervise home health aides and ensure quality patient care.
- Document care and outcomes in compliance with regulations.
- Communicate with patients, families, and healthcare providers.
Physical Demands:
- Frequent standing, walking, and lifting (up to 50 lbs).
- Exposure to patient elements, bloodborne diseases, and irregular hours.
Work Schedule:
- Monday-Friday, 8:00 AM - 4:30 PM with weekend on-call rotation.
- On-call duties every third weekend, including night and day shifts.
Additional Duties:
- Ensure patient confidentiality and culturally sensitive care.
- Participate in ongoing education and on-call duties as required.
