Full-Time Home Health RN – Daytime Hours Monday through Friday – Tecumseh, Nebraska

Connected Health Care is partnering with a respected home health agency in Tecumseh, Nebraska, to find a Full-Time Home Health RN. This is an excellent opportunity for an experienced Registered Nurse to provide compassionate care to patients in their homes while enjoying a work-life balance with daytime hours and weekends off.

Job Description:

• Provide home health care to patients, including assessing patient needs, monitoring progress, and adjusting care plans as necessary.
• Administer medications, injections, IVs, and other treatments as prescribed.
• Perform wound care, health assessments, and assist with daily activities and physical therapies.
• Collaborate with physicians, social workers, and family members to develop and implement individualized care plans.
• Monitor patient conditions and provide education on managing chronic conditions, medication use, and lifestyle changes.
• Maintain accurate patient records and document all assessments, treatments, and care plans using the EMR system.
• Weekend visits may be required on occasion to meet patient needs, though the majority of work is during daytime hours Monday through Friday.
• Ensure a safe and supportive environment for patients while following safety protocols and infection control standards.

Education & Certification Requirements:

• Active Nebraska RN License required.
• BLS (Basic Life Support) certification required.
• Home Health experience is preferred but not required; training will be provided for the right candidate.
• Previous nursing experience in a hospital, long-term care, or home health setting is preferred.

Required Experience:

• Minimum of 2 years of nursing experience, preferably with Home Health.
• Experience in wound care, IV therapy, and medication administration.
• Ability to work independently and manage a caseload of patients in a home setting.
• Excellent communication skills for working with patients, families, and healthcare professionals.
