Home Health Registered Nurse - Full-Time Day Shift

Location: Mount Vernon, OH

Connected Health Care, a Joint Commission Certified recruitment and staffing firm, is looking for a dedicated Home Health Registered Nurse for a client in Mount Vernon, OH. This full-time day shift position is ideal for compassionate nurses who excel in providing patient-centered care in a home environment.

Job Description:

• Provide skilled nursing care to patients in their homes, following individualized care plans.
• Conduct comprehensive assessments of patients' health status and develop care plans accordingly.
• Administer medications, wound care, and other nursing interventions as needed.
• Educate patients and their families on managing conditions, medication administration, and lifestyle changes.
• Collaborate with a multidisciplinary team, including physicians, therapists, and social workers, to ensure holistic care.
• Maintain accurate and timely documentation of patient care in EMR systems.
• Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.

Education + Certification Requirements:

• Associate’s or Bachelor’s degree in Nursing (BSN preferred).
• Current RN license in the state of Ohio.
• BLS (Basic Life Support) certification required.
• Prior experience in home health care is preferred.
• Strong organizational and communication skills.
