Job Description:
We are seeking a compassionate and dedicated Home Health Registered Nurse (RN) Case Manager to join our team in Jackson, California. This is a travel position, providing care to patients in the comfort of their homes. As an RN Case Manager, you will be responsible for managing a weekly case load of 25-35 patients within a 30-mile radius of Jackson.

Key Responsibilities:

• Provide skilled nursing care to home health patients in the Jackson area, including assessments, treatments, and ongoing evaluation of patient progress.

• Develop and implement patient care plans in collaboration with physicians, family members, and other healthcare professionals.

• Manage a caseload of 25-35 patients per week, ensuring high-quality care and timely visits.

• Educate patients and their families about health conditions, treatment options, and lifestyle modifications.

• Coordinate with multidisciplinary teams to optimize patient outcomes and ensure continuity of care.

• Ensure compliance with all relevant healthcare regulations and documentation standards.

Qualifications:

• Active Registered Nurse (RN) license in California.

• At least 2 years of experience in home health nursing or case management is preferred.

• Excellent communication, organizational, and time-management skills.

• Ability to work independently, manage a caseload, and travel within the designated area.

• Valid driver's license and reliable transportation for patient visits.
