Travel Home Health Registered Nurse (RN) - Bala Cynwyd,PA
Weekly Compensation: Up to $2,730/week (based on experience)

Connected Health Care is seeking a compassionate and experienced Travel Home Health Registered Nurse for a 13-week assignment in Bala Cynwyd, PA. This is a high-paying opportunity for RNs with strong OASIS and Case Management experience who thrive in independent, patient-focused environments.

Job Description:

  • Provide skilled nursing care to patients in their homes, following individualized care plans

  • Conduct initial assessments, develop care plans, and update as needed based on patient progress

  • Complete accurate and timely documentation using OASIS

  • Manage a case load efficiently while coordinating with interdisciplinary teams

  • Educate patients and families on health conditions, medications, and home safety

  • Monitor patient conditions and report changes to physicians or care managers

Education & Certification Requirements:

  • Active Pennsylvania RN license (or compact eligible)

  • Minimum of 2 years of recent home health nursing experience required

  • OASIS and Case Management experience required

  • Current AHA BLS certification

  • Valid driver's license and reliable transportation

  • Excellent clinical judgment, time management, and interpersonal skills
