Job Description:

• Patient Support: Provide emotional and social support to patients and their families.
• Care Coordination: Collaborate with medical staff to develop and implement patient care plans.
• Resource Management: Assist patients in accessing community resources and support services.
• Crisis Intervention: Offer crisis intervention and counseling services to patients and families in distress.
• Discharge Planning: Coordinate discharge plans to ensure smooth transitions from hospital to home or other care settings.
• Documentation: Maintain accurate and thorough patient records and documentation.
• Advocacy: Advocate for patient rights and access to necessary services.

Education and Certification Requirements:

• MSW Degree: Must have a Master's degree in Social Work from an accredited graduate school.
• Experience: Minimum of two (2) years of post-graduation experience in a medical setting.
• License: Current clinical license to practice in the State of Montana; LCSW is preferred.
• Skills: Strong knowledge in the principles and practices of social work, counseling, and the ability to work effectively with diverse populations.
• Organization: Must be organized and proficient in completing documentation.
• Communication: Excellent verbal and written communication skills.
• Driver's License: Must have a current Montana Driver's License.
