Social Worker (Hospital) - Globe, AZ
Full time - 10K sign on bonus
Connected Health Care, a leading recruitment and staffing firm, is partnering with a prominent hospital in Globe, AZ, to seek a compassionate and skilled Social Worker to join their dedicated healthcare team. This is an exciting opportunity to make a meaningful impact on the lives of patients and families in a vibrant and supportive hospital environment. As a Social Worker, you will play a pivotal role in providing comprehensive support services to individuals in need.
Key Responsibilities:
- Conduct thorough assessments to determine the psychosocial needs of patients and their families.
- Collaborate with interdisciplinary teams to develop and implement effective treatment plans.
- Provide counseling, emotional support, and crisis intervention to individuals and families.
- Facilitate connections with community resources, including referrals for housing, financial assistance, and healthcare services.
- Advocate for patients' rights and help navigate complex healthcare systems.
- Document all interactions and interventions accurately and maintain confidentiality.
Qualifications:
- Master's degree in Social Work from an accredited institution.
- Current licensure or eligibility for licensure as a Social Worker in the state of Arizona.
- Minimum of two years of experience in a healthcare or hospital setting is preferred.
- Strong understanding of medical terminology and knowledge of social work practices and ethics.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work collaboratively in a fast-paced, multidisciplinary team environment.
- Proficiency in using electronic medical records systems and other relevant software.
