Connected Health Care is partnered with a facility near Beatrice, Nebraska searching for a full time LPN to join their nursing team. This is a small critical access facility, looking for an individual to come in and be a strong addition to their team. This facility is open to new grads and experienced candidates! 

Job Summary:
 

1. Patient Care: Assist with providing direct patient care, including administering medications, dressing wounds, monitoring vital signs, and assisting with activities of daily living (ADLs) such as bathing, feeding, and mobility.

2. Nursing Assessments: Conduct basic patient assessments, including gathering patient health histories, noting any changes in condition, and reporting observations to the nursing team.

3. Medication Administration: Administer medications and treatments as prescribed by physicians or under the direction of registered nurses.

4. Patient Monitoring: Monitor patients for changes in their condition and report any concerns or abnormalities to the nursing staff.

5. Collaboration: Work collaboratively with registered nurses, doctors, and other healthcare professionals to develop and implement individualized patient care plans.

6. Documentation: Accurately document all patient care activities, assessments, and interventions in the electronic medical records (EMR) or other hospital documentation systems.

7. Patient Education: Provide patients and their families with education on various health topics, including disease management, medication instructions, and discharge planning.

8. Infection Control: Adhere to strict infection control protocols to maintain a safe and sterile environment for patients and staff.

9. Equipment and Supply Management: Ensure that medical equipment and supplies are well-maintained and properly stocked for patient care.

10. Emergency Response: Be prepared to respond quickly and effectively in emergency situations, such as code blue or rapid response calls.

11. Support for Families: Offer emotional support and information to patients' families, answering questions and providing reassurance as needed.

12. Compliance: Follow hospital policies, procedures, and regulatory guidelines to ensure patient safety and maintain compliance with healthcare standards.

13. Shift Handover: Provide concise and accurate handover reports to the incoming nursing staff at the end of each shift.

14. Patient Advocacy: Advocate for patients' needs and rights, ensuring they receive compassionate and appropriate care during their hospital stay.

Qualifications : 

• LPN License Nebraska 
• CPR Certification
