Job Responsibilities
- Provide direct patient care, including bathing, dressing, and feeding residents in accordance with facility protocols.
- Monitor patient health status by observing and recording vital signs and reporting any changes to the nursing staff.
- Assist in patient mobility, transfers, and positioning to ensure safety and comfort.
- Document all patient activities and care provided accurately and promptly in the EMR system.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans.
