Licensed Practical Nurse (LPN) – Long-Term Care
Location: Trimont, MN
Schedule: Mix of Days & Evenings
Pay: Up to $45/hour (Based on Experience)

Connected Health is currently hiring a dedicated and compassionate Licensed Practical Nurse (LPN) for one of our trusted long-term care partner facilities in Trimont, Minnesota. Position Details:

  • Location: Trimont, MN

  • Setting: Long-Term Care Facility

  • Schedule: Mix of Day and Evening shifts

  • Pay: Up to $45/hour, based on experience

Key Responsibilities:

  • Provide hands-on nursing care under the supervision of an RN

  • Administer medications and treatments according to resident care plans

  • Monitor residents’ conditions and report changes promptly

  • Document care accurately and follow facility protocols

  • Collaborate with staff to ensure residents receive high-quality care

Requirements:

  • Active LPN license in the state of Minnesota

  • Current BLS certification

  • Prior experience in long-term care preferred

  • Strong communication and clinical skills

  • Availability for day and evening shifts as scheduled
