School-Based Licensed Practical Nurse (LPN) – 2025/2026 School Year
Location: Worcester, MA
Schedule: Full time (31.25 hrs/week) | August 25, 2025 – June 18, 2026
Pay Rate: $35/hr

Connected Health Care, a leading provider of educational staffing solutions, is seeking a compassionate and experienced Licensed Practical Nurse (LPN) for a school-based, 1:1 role in Worcester, MA. This is a rewarding opportunity to provide direct care and support to a 6th grade student throughout the school day and during transportation.

Position Highlights:

  • 1:1 care for a 6th grade student with medical needs

  • G-tube care required

  • Bus riding required – morning and afternoon transportation

  • Work within a supportive school setting focused on student success

  • Consistent 31.25 hours/week for the full academic year
