As an LPN, you will perform patient assessments as designated by a registered nurse or provider to include the collection of data regarding the patient’s physical, psychosocial, and health history. In collaboration with a Registered Nurse or provider, you will organize patient data into the formulation of a plan of care.

During your first few weeks in this LPN role, you can expect to begin work on some of the following:

• Appropriately evaluate the developmental age of the patient when performing assessments, treatments, and patient/family education
• Carry out a plan of care to achieve desired patient outcomes
• Ensure that the care provided is appropriate to the severity of the disease, condition, impairment, or disability
• Oversee activities of ancillary and support team members
• Document patient care and concerns in the patient record
