Job Requirements
- Education Requirements: Valid Registered Nurse (RN) license in Colorado
- Certifications Requirements: Basic Life Support (BLS), CPR certification, And any state-specific certifications required for hospice nursing
- Experience Requirements: Minimum of 1 year of hospice or palliative care nursing experience, 2 years of nursing experience preferred, Experience with admissions or case management is a plus
- Other Requirements: Ability to work independently, strong communication and interpersonal skills, physical ability to lift and move patients, proficiency with EMR systems, compassionate and patient-focused attitude

Job Responsibilities
- Provide comprehensive hospice nursing care to terminally ill patients in accordance with state and federal regulations.
- Collaborate with interdisciplinary team members to develop and implement personalized care plans for patients.
- Maintain detailed and accurate documentation of patient assessments, treatments, and progress using Netsmart EMR system.
- Execute patient admissions and assessments efficiently, aiming for approximately 3 admissions per day.
- Ensure compliance with all safety protocols, policies, and hospice standards, providing compassionate support to patients and their families.
