Full-Time Licensed Practical Nurse (LPN) – Brain Injury & Short-Term Rehab – Northern Illinois
$15,000 Sign-On Bonus | Up to $40/hr (Based on Experience)
Connected Health Care is a leading healthcare recruitment and staffing firm partnering with a respected facility in Northern Illinois to hire a compassionate and skilled Full-Time LPN for their Brain Injury and Short-Term Rehab unit. This is a great opportunity for an LPN who is passionate about rehab, patient recovery, and long-term outcomes.
Position Overview
As a Full-Time LPN in this Brain Injury and Short-Term Rehab setting, you will play a key role in helping patients regain independence and quality of life. You’ll collaborate closely with RNs, therapists, and providers to deliver patient-centered, evidence-based care in a supportive, team-focused environment.
Key Responsibilities
- Provide direct nursing care to patients with brain injuries and those in short-term rehabilitation
- Administer medications and treatments in accordance with facility policies and physician orders
- Monitor vital signs, changes in condition, and promptly report to the RN/physician as needed
- Assist with wound care, ADLs, and rehabilitation-focused nursing interventions
- Support patients with mobility, transfers, and safe ambulation as they progress through rehab
- Document assessments, interventions, and outcomes accurately and timely in the medical record
- Collaborate with an interdisciplinary team including PT/OT, SLP, social work, and case management
- Educate patients and families on care plans, recovery expectations, and discharge planning
- Maintain a safe, clean, and compliant environment following all infection control standards
- Promote a positive, patient-first culture and uphold high standards of professionalism
Schedule & Compensation
- Full-time position (primarily 36–40 hours/week; schedule details discussed in interview)
- Competitive hourly rate up to $40/hr based on experience
- $15,000 sign-on bonus
- Shift differentials may be available depending on schedule
- Comprehensive benefits package (details provided by facility during interview)
Qualifications
- Active LPN license in the state of Illinois (or eligibility to obtain prior to start)
- Minimum of 1 year of nursing experience preferred; rehab, skilled nursing, or acute care experience a plus
- Experience or strong interest in brain injury care and short-term rehab
- Strong clinical assessment, critical thinking, and time-management skills
- Excellent communication and teamwork abilities
- Compassionate, patient-centered approach with a commitment to quality and safety
Education & Certification Requirements
- Graduate of an accredited Practical Nursing program
- Current, unencumbered LPN license in Illinois
- Current BLS (Basic Life Support) certification from the American Heart Association (or willing to obtain prior to start)
- Additional certifications in rehabilitation, wound care, or neurological care are a plus but not required
