Job Description:

• Provide compassionate and comprehensive nursing care to hospice patients in various settings, including homes, assisted living facilities, and skilled nursing facilities.
• Perform patient assessments, develop individualized care plans, and coordinate care with interdisciplinary team members to ensure the highest quality of care for patients and their families.
• Administer medications, manage symptoms, and provide education and emotional support to patients and families regarding end-of-life care, pain management, and comfort measures.
• Document patient assessments, interventions, and outcomes accurately and in a timely manner using electronic medical records (EMR) systems.
• Collaborate with physicians, nurse practitioners, social workers, and other healthcare professionals to optimize patient outcomes and promote interdisciplinary teamwork.
• Participate in team meetings, case conferences, and educational opportunities to enhance clinical skills and stay updated on best practices in hospice care.

Requirements:

• Valid Registered Nurse (RN) license in the state of Colorado.
• Minimum of one year of nursing experience in hospice or palliative care preferred.
• Current CPR certification.
• Strong assessment, communication, and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Proficiency in using electronic medical records (EMR) systems.
