Travel Licensed Practical Nurse (LPN) – Northern Illinois

Connected Health Care – LTC / Rehab (Neuro Rehabilitation)
Up to $1,700/week | 13-Week Contract | Day or Night Shift | 3x12s | 36 Hours

Connected Health Care, a premier recruitment and staffing firm, is seeking compassionate and dedicated Travel Licensed Practical Nurses (LPNs) to support our partnered long-term care and rehabilitation facility in Northern Illinois. This not-for-profit rehabilitation center specializes in care for young adults with traumatic brain injury, spinal cord injuries, and other neurological disorders.

This assignment is ideal for LPNs who enjoy building meaningful patient relationships while gaining hands-on experience in a specialized neuro-rehab setting.

Position Details

  • Contract Length: 13 weeks

  • Schedule: Day or Night Shift

  • Hours: 3x12s | 36 hours/week

  • Setting: LTC / Rehab — Neuro & Complex Care

  • Compensation: Up to $1,700 per week

  • Start Date: ASAP or upon credentialing

Job Responsibilities

  • Deliver compassionate, high-quality nursing care to patients recovering from neurological injuries and conditions.

  • Monitor patient status and report changes to the RN and provider team.

  • Administer medications and treatments per physician orders and facility protocols.

  • Assist with activities of daily living (ADLs) and rehabilitation support.

  • Collaborate with interdisciplinary rehab teams including nursing, PT, OT, SLP, respiratory therapy, and behavioral health.

  • Accurately document patient care and progress.

  • Maintain compliance with all facility policies and state and federal regulations.

Education & Certification Requirements

  • Active Licensed Practical Nurse (LPN/LVN) license — Illinois or compact state.

  • Minimum 1 year of LPN experience — rehabilitation, neuro, TBI/SCI, or LTC experience preferred.

  • Current BLS certification (AHA).

  • Strong communication, teamwork, and documentation skills.
