Job Requirements
- Education Requirements: Bachelor's degree in Nursing or related healthcare field
- Certifications Requirements: Valid Advanced Practice Nurse license, Wound Care Certification, BLS certification
- Experience Requirements: Minimum of 1 year of wound care experience, experience with amniotic wound care preferred, previous home health experience is a plus
- Other Requirements: Ability to work independently, excellent communication skills, valid driver’s license, reliable transportation, soft skills including compassion and attention to detail

Job Responsibilities
- Provide expert wound care management for patients with amniotic ulcers, diabetic wounds, pressure injuries, venous ulcers, and other chronic wounds.
- Assess patient conditions regularly, develop and implement individualized treatment plans, and adjust interventions as necessary to promote healing.
- Educate patients and family members on wound care procedures, prevention strategies, and post-treatment care to ensure optimal outcomes.
- Maintain accurate and thorough documentation of patient assessments, treatments, and progress in electronic medical records.
- Collaborate with multidisciplinary teams, including physicians, home health staff, and other healthcare professionals, to coordinate care and ensure comprehensive patient support.
