Connected Health Care, a leading recruitment and staffing firm, is seeking an experienced Travel Home Health Registered Nurse (RN) Case Manager for a 13-week travel assignment in Bala Cynwyd, Pennsylvania. Contract Length: 13 Weeks
Compensation: Up to $2,730 per week (based on experience)
Weekend Requirement: 1 weekend in 4
Job Description:
As a Travel Home Health RN Case Manager, you will provide skilled nursing care, case management, and coordination of services for patients in their homes. You will work closely with an interdisciplinary team to ensure quality care and optimal patient outcomes.
Responsibilities:
- Manage a caseload of home health patients, performing comprehensive assessments and developing individualized care plans
- Provide direct patient care, including medication administration, wound care, and chronic disease management
- Monitor and document patient progress while coordinating care with physicians, therapists, and family members
- Educate patients and families on treatment plans, medications, and at-home care
- Maintain accurate and timely clinical documentation in accordance with agency and regulatory standards
- Participate in weekend rotation (1 weekend in 4) as scheduled
Education and Certification Requirements:
- Active Registered Nurse (RN) License in the state of Pennsylvania (or compact license)
- BLS Certification through the American Heart Association required
- Minimum of 1 year of recent Home Health experience preferred
- Knowledge of OASIS documentation and Home Health regulations strongly preferred
Skills and Qualifications:
- Excellent communication and critical thinking skills
- Strong attention to detail and ability to work independently
- Compassionate and patient-centered approach to care
- Reliable transportation for home visits
