Job Requirements
- Education Requirements: Bachelor’s degree in Nursing (BSN) or higher from an accredited institution
- Certifications Requirements:
- Valid Advanced Practice Registered Nurse (APRN) license in Wisconsin
- Basic Life Support (BLS) certification
- Wound Care Certification (e.g., CWCN or WCC) preferred
- Experience Requirements:
- Minimum of 1 year of experience in wound care, including amniotic wound care, diabetic, pressure, venous, and chronic wound treatment
- Experience working in home health settings
- Proven ability to manage on-site patient care efficiently
- Other Requirements:
- Excellent communication and interpersonal skills
- Ability to work independently and adapt to changing patient needs
- Current valid driver’s license and reliable transportation

- Provide expert wound care management, including amniotic wound therapy, diabetic, pressure, venous, and chronic wound treatment to home-bound patients
- Assess patient conditions, develop individualized treatment plans, and execute wound care protocols according to best practices
- Collaborate with physicians, caregivers, and interdisciplinary teams to ensure optimal patient outcomes
- Document all patient interactions, treatments, and progress accurately in electronic medical records (EMR)
- Educate patients and families on wound management, prevention strategies, and necessary follow-up care
