Travel LVN – Skilled Nursing Facility (SNF) – Portola, CA
13-Week Contract | Up to $2,520/Week

Connected Health Care, a leading recruitment and staffing firm, is currently seeking a compassionate and reliable Licensed Vocational Nurse (LVN) for a 13-week travel contract at a Skilled Nursing Facility (SNF) in Portola, California. This opportunity is ideal for LVNs who are passionate about senior care and want to enjoy the charm and outdoor beauty of the Northern Sierra Nevada region.

Position Overview:

  • Location: Portola, CA

  • Contract Length: 13 weeks

  • Compensation: Up to $2,520/week, based on experience

  • Setting: Skilled Nursing Facility (SNF)

Job Responsibilities:

  • Deliver patient-centered nursing care to residents in a SNF setting

  • Administer medications and treatments as prescribed

  • Monitor patient health and report any changes to the care team

  • Assist with daily activities including mobility, hygiene, and nutrition

  • Maintain accurate documentation and support patient care plans

  • Collaborate with RNs, CNAs, and interdisciplinary staff to ensure high-quality care

Qualifications:

  • Active California LVN license required

  • Minimum 1 year of experience in a SNF or long-term care setting preferred

  • BLS certification required (AHA preferred)

  • Strong communication and critical thinking skills

  • Empathy, patience, and dedication to providing high-quality elder care
