RN Case Manager – Nursing Home | Minnesota
We’re looking for an RN Case Manager to join a skilled nursing facility team in Minnesota!
Position Overview:
As the RN Case Manager, you’ll oversee resident care plans, coordinate between medical staff and families, and ensure residents receive the best possible care and services. This is a key role that combines clinical knowledge with strong communication and organizational skills.
Key Responsibilities:
- Develop and manage individualized care plans for residents
- Coordinate care between nurses, therapy, physicians, and families
- Conduct regular assessments and update care plans as needed
- Ensure compliance with facility policies and state regulations
- Support discharge planning and transitions of care
- Provide guidance and leadership to nursing staff as needed
Requirements:
- Active RN license in Minnesota (or compact state license)
- Experience in long-term care, skilled nursing, or rehabilitation preferred
- Strong communication and organizational skills
- Compassionate and resident-focused approach
Schedule & Pay:
- Full-time, day shift (typical Monday–Friday schedule)
- Competitive pay based on experience
