- Provide comprehensive home health and hospice nursing care to patients in their homes, ensuring compliance with healthcare standards and protocols.
- Assess patient conditions, develop individualized care plans, and modify treatments based on patient progress and needs.
- Collaborate with interdisciplinary teams, caregivers, and family members to optimize patient outcomes and ensure quality care.
- Maintain accurate and detailed patient documentation, including charts, progress notes, and care plans, in electronic health record systems.
- Execute all duties in accordance with facility policies, state regulations, and best practices to ensure safe and effective patient care.
