Job Description:

• Provide occupational therapy services to patients in a skilled nursing facility (SNF) and rehabilitation setting to improve their functional abilities, independence, and quality of life.
• Conduct comprehensive assessments, develop individualized treatment plans, and implement therapeutic interventions tailored to patients' needs and goals.
• Collaborate with a multidisciplinary team, including physicians, nurses, physical therapists, and speech-language pathologists, to coordinate patient care and optimize outcomes.
• Administer therapeutic exercises, activities of daily living (ADL) training, adaptive equipment recommendations, and environmental modifications to promote recovery and enhance daily functioning.
• Educate patients, families, and caregivers on therapeutic techniques, safety precautions, home exercises, and adaptive strategies to support continued progress and independence.
• Document patient progress, treatment plans, and outcomes accurately and in compliance with regulatory standards and healthcare policies.

Education + Certification Requirements:

• Master's degree in Occupational Therapy from an accredited program.
• Valid state licensure as an Occupational Therapist (OT).
• Certification by the National Board for Certification in Occupational Therapy (NBCOT).
• CPR certification.
• Previous experience in a SNF or rehabilitation setting is preferred.
