Job Requirements
  • Education Requirements: Bachelor's or Master's degree in Physical Therapy from an accredited institution
  • Certifications Requirements: Valid state licensure as a Physical Therapist, CPR certification, and Basic Life Support (BLS)
  • Experience Requirements: Minimum of 1 year of recent experience as a Physical Therapist in a long-term care or skilled nursing facility
  • Other Requirements: Ability to float between units if necessary, strong communication skills, and the ability to work independently

Job Responsibilities
  • Provide comprehensive physical therapy evaluations for long-term care residents to develop individualized treatment plans.
  • Implement and monitor therapy programs focused on improving residents' mobility, strength, and overall well-being.
  • Collaborate with the therapy team, including PTAs, to ensure effective case load management and optimal patient care.
  • Document resident progress, treatment plans, and outcomes accurately in the PCC and Net Health systems.
  • Participate in resident discharge planning and provide guidance to staff and family members regarding continued care needs.
