Job Description:

As an LPN in a Skilled Nursing Facility, you will play a crucial role in providing exceptional patient care to residents requiring short-term rehabilitation or long-term skilled nursing services. You will work closely with registered nurses (RNs), physicians, and other healthcare professionals to ensure the highest level of care and comfort for patients.

Key Responsibilities:

• Deliver high-quality nursing care in accordance with SNF protocols and patient care plans.
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor and document vital signs, symptoms, and patient progress.
• Assist with wound care, IV therapy, and other skilled nursing procedures.
• Provide emotional and physical support to residents and their families.
• Collaborate with interdisciplinary teams to create and implement individualized care plans.
• Ensure compliance with state and federal regulations, as well as infection control protocols.
• Respond to medical emergencies and implement appropriate interventions.

Education & Certification Requirements:

• Active LPN license in the state of Ohio (or compact license with eligibility to practice in OH).
• Certifications Required: Basic Life Support (BLS).
• Preferred Certifications: IV Certification, Wound Care Certification (WCC).
• Minimum Experience: 1+ year of experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting preferred.
• Strong communication, problem-solving, and patient care skills.
