Location: Alamosa, CO
Contract: 13 Weeks
Weekly Pay: Up to $2,400
Connected Health Care, a leading recruitment and staffing firm, is partnering with a top-tier Skilled Nursing Facility (SNF) near Alamosa, CO to find a dedicated Travel Physical Therapist (PT) for a 13-week contract. Job Responsibilities:
- Provide comprehensive physical therapy services to patients in a Skilled Nursing Facility
- Assess patients' mobility, strength, and physical function to create customized treatment plans
- Work alongside nurses, occupational therapists, and physicians to optimize rehabilitation outcomes
- Educate patients and caregivers on therapy techniques and home exercise programs
- Monitor and document patient progress and treatment adjustments
- Ensure compliance with state and federal SNF regulations
Qualifications:
- Doctorate or Master’s in Physical Therapy from an accredited program
- Active PT license in Colorado (or eligibility for compact licensure)
- BLS Certification (Basic Life Support)
- Previous SNF or geriatric rehab experience preferred
- Strong communication and patient-care skills
