Connected Health Care, a premier recruitment and staffing firm, is looking for a skilled Physical Therapist to join our team on a 13-week contract in a Skilled Nursing Facility. If you’re passionate about helping patients regain their mobility and improve their quality of life, this is the perfect role for you!
Position Summary: As a Physical Therapist, you will provide direct rehabilitation care to patients following physician referrals, adhering to all applicable laws, regulations, and standards. This role involves developing, assessing, and modifying individualized treatment plans to help patients achieve their goals.
Key Responsibilities:
- Deliver exceptional physical therapy services in a Skilled Nursing Facility.
- Establish, assess, and adjust treatment plans with measurable and functional goals.
- Supervise and evaluate care provided by PTAs, Rehab Aides, and PT/PTA students.
- Maintain accurate and timely patient records using therapy software.
- Collaborate effectively with interdisciplinary teams to ensure comprehensive patient care.
- Uphold professional standards and confidentiality in all interactions.
Requirements:
- Graduate of an accredited Physical Therapy program (BSPT, MSPT, or DPT required for graduates after 2003).
- Active Physical Therapy license in the applicable state.
- CPR certificationÂ
- Preferably one year of experience in post-acute care or a related setting.
- Proficient in Microsoft Word, Excel, and email communication.
- Strong interpersonal skills and ability to work independently.
