Connected Health Care, a leading recruitment and staffing firm, is seeking a dedicated Travel Physical Therapist for a Skilled Nursing Facility (SNF) located near Port Angeles, Washington. Job Responsibilities:
-
Evaluate and develop treatment plans for patients with physical impairments.
-
Implement therapeutic programs to improve mobility, relieve pain, and prevent or limit permanent physical disabilities.

Collaborate with nurses, physicians, and occupational therapists to optimize patient outcomes.
-
Document patient progress and adjust plans as needed.
-
Educate patients and families on recovery strategies and exercises.
Qualifications:
-
Active Physical Therapist (PT) license in the state of Washington
-
Graduate of an accredited Physical Therapy program
-
BLS Certification (preferred)
-
Prior SNF or geriatric experience is a plus
-
Ability to work independently and as part of an interdisciplinary team

Strong communication and organizational skills
