Job Description: We are seeking a dedicated and compassionate Physical Therapist for an immediate opening in a Home Health setting for our client in Stevens Point, Wisconsin. This is an exciting opportunity to expand your skills, explore new locations, and make a significant impact on the community!

Key Responsibilities:

• Provide physical therapy services to patients in their homes according to the physician's plan of care.
• Assess and document patient progress, adjust treatments as necessary, and communicate with healthcare team members.

• Develop and implement individualized treatment plans based on patient needs and goals.
• Ensure compliance with all healthcare regulations and standards.

Education and Certification Requirements:

• Valid and current state license as a Physical Therapist.
• Graduation from an accredited Physical Therapy program.
• Minimum of 1-2 years of relevant clinical experience.
• Previous Travel/Per diem experience preferred but not required.
