Connected Health Care is partnering with a reputable outpatient clinic near Big Bear, CA to fill the position of Outpatient Physical Therapist. We are seeking a dedicated and passionate individual to join our team and provide exceptional care to our patients.

Job Description:

• Conduct patient evaluations and develop individualized treatment plans.
• Implement and monitor therapy programs to improve patients' mobility, strength, and function.
• Educate patients and their families about therapy goals and home exercise programs.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate and up-to-date patient records and documentation.
• Participate in team meetings and continuing education to enhance clinical skills.

Requirements:

• Doctorate or Master's degree in Physical Therapy from an accredited program.
• Active California state license or eligibility for licensure.
• Certification in CPR/BLS.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a multidisciplinary team.
• Experience in outpatient physical therapy preferred but not required.
