Job Description:

• Provide physical therapy services to patients in accordance with physician orders.
• Develop and implement individualized treatment plans.
• Evaluate patient progress and adjust treatment plans as necessary.
• Communicate effectively with patients, families, and other healthcare professionals.
• Maintain accurate and timely documentation of patient care.

Education and Certification Requirements:

• Master’s degree or higher in Physical Therapy from an accredited institution.
• Valid Physical Therapist license in the state of Kentucky.
• Current BLS certification.
