- Education Requirements: Bachelor's or higher degree in Physical Therapy from an accredited program
- Certifications Requirements: Current state licensure as a Physical Therapist (PT); CPR certification; preferably BLS certified
- Experience Requirements: Minimum of 1 year of clinical experience in a long-term care setting
- Other Requirements: Ability to work flexible days and hours; strong communication skills; comfortable working with advanced EMR systems; able to float between departments if needed; physical stamina to assist residents

Job Responsibilities
- Provide comprehensive physical therapy assessments and develop individualized treatment plans for residents in the long-term care facility.
- Execute and monitor therapeutic interventions aimed at improving residents' mobility, strength, and functional independence.
- Collaborate with multidisciplinary team members, including PTAs, SLPs, and nursing staff, to ensure cohesive patient care.
- Maintain accurate documentation of patient progress, treatment plans, and clinical notes using PCC and Net Health systems.
- Assist in training and supervising PTA support to optimize patient case load management and therapy delivery.
