- Education Requirements: Bachelor's or Master's degree in Physical Therapy from an accredited program
- Certifications Requirements: Current California state licensure, BLS certification, Missouri and/or NBCOT Certification (if applicable)
- Experience Requirements: Minimum of 1 year of outpatient or skilled nursing facility experience in a clinical setting
- Other Requirements: Strong communication skills, ability to work independently, proficiency with Electronic Medical Records (EMR) systems, and flexibility with schedule

Job Responsibilities
- Provide high-quality physical therapy services to patients in skilled nursing and long-term care facilities, assessing individual needs and developing personalized treatment plans.
- Collaborate with interdisciplinary teams including PTA, nursing staff, and physicians to coordinate patient care and optimize recovery outcomes.
- Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans within EMR systems such as PCC and Net Health.
- Educate patients and their families on therapeutic exercises, mobility techniques, and injury prevention strategies.
- Adapt therapy approaches based on patient progress, census fluctuations, and evolving medical conditions to ensure optimal care delivery.
