Contract/Travel Physical Therapist

Location: Wisconsin

Setting: Mixed - Inpatient Acute, Swing Bed, Outpatient, Skilled Nursing Facility (SNF) (Weighted more heavily towards outpatient; Current staff willing to adjust load as needed/desired)

Start Date: ASAP

Schedule: Monday to Friday, 40 hours guaranteed. Flexible hours, mainly 8:00 AM - 4:30 PM, but will accommodate adjustments.

Case Load: 8 patients per day

Culture: Small facility in a rural setting, fostering a supportive and collaborative environment. No strict productivity standards. Physical Therapists work closely with in-house providers.

Job Description:

Our facility in Wisconsin is seeking a dedicated and experienced Contract/Travel Physical Therapist to join our team. As a Physical Therapist in our mixed setting, you will have the opportunity to work in various environments including inpatient acute care, swing bed, outpatient, and skilled nursing facility (SNF), with a focus on outpatient care. We prioritize patient care and believe in giving our therapists the flexibility to provide individualized treatment plans without strict productivity constraints. Responsibilities:

• Evaluate, plan, and administer comprehensive physical therapy modalities/regimens to patients in a variety of settings.
• Develop and implement individualized treatment plans based on patient needs and goals.
• Collaborate closely with in-house providers to ensure coordinated and effective patient care.
• Maintain accurate and up-to-date patient records.

Requirements:

• Valid Physical Therapist license in the state of Wisconsin.
• Experience in a mixed setting, particularly outpatient care, is preferred.
• Strong clinical and interpersonal skills.
• Ability to work independently and as part of a collaborative team.
• Excellent communication and documentation abilities.
