Job Description:

• Provide exceptional physical therapy services in a skilled nursing facility (SNF) setting, encompassing both inpatient (IP) and outpatient (OP) care.
• Develop and implement individualized treatment plans to improve patients' mobility, strength, and functional abilities.
• Conduct evaluations and assessments to determine patients' needs and goals.
• Collaborate with interdisciplinary teams to optimize patient outcomes and ensure continuity of care.
• Document patient progress accurately and maintain detailed records of treatment interventions.

Education and Certification Requirements:

• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• State licensure as a Physical Therapist in Nebraska.
• Minimum of 1+ years of experience working in a skilled nursing facility with both inpatient and outpatient settings.

Contract Details:

• Contract Length: 13 weeks
• Weekly Compensation: Up to $2300 based on experience
