Connected Health Care is currently seeking a passionate and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our esteemed client, a skilled nursing facility and rehabilitation center located north of Boise, ID. Job Description:

• Provide comprehensive nursing care to residents in the skilled nursing facility/rehabilitation center.
• Administer medications and treatments as prescribed by physicians.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Monitor and assess residents' health status, documenting changes and reporting concerns to appropriate healthcare professionals.
• Educate residents and their families on healthcare needs and available resources.

Education and Certification Requirements:

• Valid RN or LPN license in the state of Idaho.
• Graduation from an accredited nursing program.
• Current CPR certification. 
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.
